Documents from a Form - Set Up

• To enable documents to be selected from within a form, the document widget must be in the form's template. If you have used "Start a Form from Documents" previously, you will still need to update this widget to select which files are available. 
• NOTE: PDF files in Company Documents are strongly recommended!

1. From your main SafetyLoop screen, click "Forms" then click "Template Creator". 

2. Located the applicable form template, then click the action dots to the right of it, then click "Edit". 

3. Once in the template, locate where you would like to add the new document widget. 

• If adding a new question, click into the text box of another question to add a new one BELOW it. 
• Once you have clicked into the text box, some tabs will appear. Click the "+Question" tab to the left of that question.
• The new question will appear below it, add your text then follow the next step. 

• Once you have located your new question, or the one you need to update. Click the "Answer Type" dropdown to the right of the text box. 
• NOTE: This might say "Document" if you are simply updating an existing question! 

4. Once you've clicked the answer type dropdown shown above, click the "Document" widget.

5. This will then prompt a document window where you can select all the documents you would like to be selectable from this template. Once you've selected all files, click "SAVE AND APPLY". 

• You can check entire folders, or use the "+" icon to expand the folder and check mark individual files. 
• Once check marked, the files will show above as selected. 

6. You will then be brought back to the template in edit, you can use the back arrow in the upper left-hand corner to exit the template. 

Users will now be able to select the document from a dropdown list within their form! 

• NOTE: It is best to use PDFs in your company documents as this is best for previewing the file. Other formats may not support this. 
• Android devices often require a PDF download and do not support PDF preview, it is best to download a reliable PDF viewer in these cases!
